Let's just start out by saying this is one of the sickest, foulest real movies I have ever seen.  I say real movies because long ago I stopped counting all the non-plot, non-story line just sick to be sick films.  I really dislike just sick to be sick.  Do not insult my artistic intelligence with something only put together for the cliche' "hardcore extreme cinema" lovers out there.  Give me a script with dialogue, a story, and preferably a good one.  Is this a good one?  Eh I wouldn't say it was real original but what is original about this one is the outright nastiness of it being from Belgium in the 80s which at the time was a country that hadn't churned out much in the horror genre.  In comes Johan Vandewoestijne to make everyone puke and lose their minds at one of the most horrific and disgusting psychopath films of all time.

We have Lucker played by Nick Van Suyt who rivals Joe Spinell for raunchiest looking maniac in an 80s movie.  I wouldn't doubt at all that Maniac was indeed one of the main inspirations for this movie being that they are so similar in story line and the shape and size of the killer haha.  Plus I feel that the murderer in both had severe mental issues which lead them both to becoming the monsters that they are.  One tiny difference though, Lucker has sex with some of his dead victims.  In one scene in particular which I will never forget seeing the actor's bare white ass rise up and down on a dead rotting corpse.  Just plain puke.  Lucker escaped from a mental institution where he was serving time for trying to slit his own throat after killing several women and having sex with their dead bodies.
So Lucker kills the doctor and a nurse in the hospital and heads off to continue his

killing spree in a very bloody and gory fashion.  Stabbing one guy through the eye, one victim gets their head bashed, a prostitute is hand cuffed to a bed and murdered in a gory sexual display of disgust.  As far as fx goes I have to say that given where this film came from they are damn good.  Really get the point across and really get the crude sickness of the killer across in a big way.  Something no matter what anyone says is necessary in a film like this.  Cutting out gore and blood never makes a horror film better in this reviewers opinion.  I highly recommend this film for any fan of extreme cinema.  Lost for a period of time the director had to piece this "Director's Cut" together using the only remaining video footage he had at his disposal since the original film's negatives were destroyed for some unknown reason.
